Educating patients on how to properly take their medications.
Chronic disease management (e.g., diabetes, hypertension, COPD). Assisting patients in managing chronic conditions like diabetes, heart disease, respiratory diseases (e.g., COPD, asthma), and kidney disease.
Monitoring symptoms, adjusting care plans as necessary, and helping patients with lifestyle changes like diet and exercise.
Post-surgical recovery support Assisting patients with recovery after surgery, ensuring proper healing and monitoring for any complications like infections or blood clots.
